We ran into this issue before our inspection by JCAHO and the decision we reached was to use the face photo and daily ask the patient to state their name and date of birth before treatment as a second identifier.

We have an Impac system as well and use the photo i.d. as a pt. identifier.
There has not been any discussion per Joint Commission that we have to
change this rule as a pt. identifier.I can not see the efficiency in
armbanding every pt. daily for tx. naome.gideon@st-johns.org

    I work in RadOnc now; in the facility that I work in; if moderate sedation is used; RN must be ACLS certified. If you do procedures w/o sedation; no problem. Our radiation therapists connect catheters to HDR afterloader; we do not insert any contrast in balloon; therapists or physician does. You do not do anything to the ports at all (it is only accessed to add contrast or removing; you clean site once/daily, (we do after pm tx); because of increase chances of dislodging balloon. Nurse or therapist can assemble ovoid devices; only physician inserts. We are still working on P&Ps.

    I am a Hem-ONC / Chemo RN who has moved into the role of a radiation therapy
    nurse in a hospital based cancer center in Georgia. We have a nurse manager
    who is looking at processes & policies. I would like to know the policies &
    procedures in other hospital based cancer centers as they relate to the care
    of patient receiving brachytherapy. We do a lot of contura balloon HDR tx(
    mammosite ) & vaginal cylinder & T & O HDR tx. Do you give any conscious
    sedation for your procedures? A discussion & difference of opinion as come
    up as to the nurses role in these procedures. I have been told nurses are
    not to touch the mammosite catheters or connect the cables - this is the
    physicists role. I would like to know what other nurses do as it relates to
    the care of these patients. Do you aspirate or attempt to aspirate any
    fluid from the vacume port of the catheters? Do you ever instill any conray
    into the catheter balloons? Do you only do dressing changes after the tx? Do
    you connect cables to any of the HDR applicator devices or delivery units?
    Do you assemble the Ovoid devices for your physicians to insert for your T &
    O HDR's? ( We don't insert the devices the physician does & determines what
    sizes to use )Does anyone have any policies & procedures they can share with
    me. My physicians & physicists say these are normal nursing duties in the
    Rad ONC setting but my nurse manager says they go beyound the nurses scope
    of practice. I have not had the opportunity to visit any other RAD ONC
    sites for any comparison & as the only RN in the department I do not see
    that happening any time in the near future. I am courious as to what is done
    at other sites & considered normal duties.
